I found a coffee mug that said, "I love my daddy" and brought it to my husband at work. You could also find picture frame that says, "I love my grandma" (or I love my daddy) and give it with a little note that says, "picture coming soon" or with a photo of your first ultrasound.
Wow, a pregnant grandmother, that's going to be a big surprise, no matter what! I think he'll be more suprised about the grandmother part!
I did this for the grandparents---after my very first ultrasound, I mailed them the picture of my baby. I attached a note that said, " Can't wait to see you! Love, baby" For my husband, I had a candy basket delivered to his work full of "Congrat's on the baby!" candy cigars. He was totally surprised and excited. Maybe this will help you.
After sex: "Guess what?? You´re doing a pregant grandma!"
How about buying a baby rattle and putting it under the covers on his side of the bed, so when he gets into bed at night and pulls up the covers he discovers it. As for grandma, I would mail her one and wait for her to call. How cool!
Or maybe you could serve him dinner on a kids plate, you know, with the cute little character plates and the tiny fork. Or just set the table for 3, two adult settings and one child one.

When we were pregnant with our first, we found out not to long before Christmas, and after the first Sonogram, my husband scanned the sonogram, then kinda drew a Santa hat on the image of the head, and we did those as christmas cards and sent them to all our family it was a cute way to announce, not sure how you could incorporate that with Valentines maybe... for the grandma anyway.
