This happened to my friend this past year. Her husband went back to the doctor for a 3rd. test (after they discovered the pregnancy) and they discovered that his vas. had grown back together on one side. He had to go back and have the vasectomy redone.
That is very rare. Are you sure that he is the father?
It's very rare, but it does happen. The failure rate of a vasectomy is 0.1% (so 1/1000 will become pregnant in any given year).
