Can't get and birth record for my self because I have two last names theone I use and the one on my birth cert

my mother last name is on my birth certified but all my life I have been using my fathers last name now I need to know what do I need to get and birth certified

Answer:
go to the county court house and ask where the county clerk office is. tell them ur birth name and they will give u a copy right there and then. if u need a birth record for something they will only use ur legal name (like insurance, child support, court case). u should change ur name legally if u want to use the name u've been calling urself. the county clerk can help u with this also
I had a similar problem. I had always used my father's last name (who died when I was 10), and then when it came to me becoming 16, I had to get a copy of my birth certificate to get my license. Low and behold, my father was listed as my mom's ex-husband and so was my last name (she wasn't with her ex when she had me, the divorce just hadn't finalized yet). Anyhoot, so I had to get my license in that last name. I was pissed off because she knew about it for years and did nothing. After I turned 18, I petitioned the court and amended the name on my birth certificate so I could use my given name (from my father) on my license and other documents. It was cheaper to do it myself then to use a lawyer, but if you're not too smart, a lawyer is the best best (I'm not saying you're an idiot... it just can be confusing for some people). I did the amendment and petition in Oklahoma, but you might have different state laws. I first petitioned the court, put a legal public notice in the paper as required by law, then spoke to the judge and he granted my petition. Then, I had to take the paper to the tag agency (which is where we got license's) and they created a duplicate license with the new name on it. in all, I spent around $200 - $300. It was $30 for me to amend my birth certificate. It was $15 for the amendment and $15 for them to send me a copy.

PS: My case/petition was easy because it was uncontested (that's why you put the legal notice out, so if people contest it, they can show up at court). Since I'm guessing you're a minor, maybe your mom would contest it? If not, you can ask your dad to file in the court on behalf of you.
The person at the Vital Statistics office should be able to tell you that.
You can call yourself anything you like, but for official records, you need to use your offical last name. That would be whatever is on your birth certificate unless its been changed legally. And by the way, any other last names on things like drivers licence, credit cards, etc. will invalidate the documents - means you can be busted for not having proper licences and documentation.
u don't have i.d.? they usually want ur birth certificate and they put what's on ur certificate as ur name on ur i.d. or social security even, it should have ur birth name because thats ur legal name.
First of all - did you have your name legally changed or did you just start using your dad's name? You may have to get legal documentation (like your medical records) that shows both your birth name and your used last name. Check your state but usually if you use a name for so long you can get it legally changes pretty easy.
