When my son wipes his tush there is blood on the toilet paper, should i be concerned?

Bright red usually means something more on the skin's surface, maybe he wiped a little too hard (you can get flushable wipes in any drug store, they are great for after a bowel movement). Darker red/black indicates some sort of intestinal/kidney/internal problem. If it were his appendix his urine would be bloody. If it is bright red I'd wait before calling the doctor, if your son is not in any discomfort there isn't much they would do for him except examine him and that could be embarssing for him. If it continues make an appointment with the doctor.

Take him for a doctor's check up if this continues. If this continues it would be important to have a doctor rule out a gastrointestinal bleed which could be dangerous esp at a young age.

However, it may just be that his bowel motion is hard and he needs to eat more fibrous foods as well as drink more water to soften his stools, so it doesn't strain him and cause a small PR bleed when he empties his bowels.

My daughter was constipated and had a really hard stool, which in turn made her bleed. I called her pediatrician who told me it was fine... and she was fine. I make sure she has raisins in her diet now to help her go more.

Don't freak out, I have three children and all of them had the same thing. You should let their doctor know just in case it is something serious, but most likely it is because their stool is tough and causes little lacerations.
Could be nothing more than being to rough when wiping. Keep an eye on it and if it gets worse, take him to the doctor. Sometimes it happens because they strain to hard when going. I'm mom to 3 and have seen this before.
