Just ask up front if e-mailing is a preferred method of communication with her/him. It's preferable to notes home because the notes can get lost en-route. Phone calls can be tough because either it's the middle of the school day and the teacher is busy or you must wait until after hours and that's not fair on the teachers (when does their work day end?). I'll best most will welcome the method.
Be casual and just ask if you can contact your teacher by e-mail when you need help on something.
I'd ask the teacher something like, "What's the best way to contact you if I have any questions?" Then if they say e-mail, you can ask for the e-mail address.

You can either ask the teacher for it, or call the school secretary and ask them. You may even be able to find it on the school's website. All teachers should have an e-mail account set up by the school.
Our school district has its own web site and all of the teachers emails are listed. If your school doesnt provide this, I would call the teacher directly and ask them if they mind emails being sent to ask questions or check up on the student.
Something that is that personal is better asked for in person or over the phone. I would say if you have a specific reason for wanting it then it shouldn't be a problem for them. Notes from home tend to get lost, and are too impersonal.
I suppose if the teacher is happy to be emailed then okay but I would rather talk to the teacher. I guess these days though with working parents they can't get to the school and this is a great way to keep in touch with the teacher and voice their concerns and vise versa. I would not ask the teacher for her email address I don't think that is right but if it is offered it would be handy for many people. But never ask. Teachers already spend hours a night at home preparing for the next day without having to respond to emails from the parents
